Healthcare providers encourage families to remain current with their immunization schedule and offer well visits to administer vaccinations. The state of Florida requires students entering, repeating, or transferring into 7th grade to provide documentation for the tetanus-diphtheria-pertussis (Tdap) vaccine. This requirement is in addition to the grade-appropriate immunization requirements.

Please review this letter and return the appropriate forms to the A.D. Henderson front office by August 13, 2024.
