National School Lunch Program & Reduced Activity Fee Requirements

Your initial 2024-2025 school year activity fee payment is based on your family’s 2023-2024 application status. To qualify for the National School Lunch Program, parents and guardians must complete a new National School Lunch Program (NSLP) application every year; this application opens on July 1. Qualifying meal status through this program also determines other benefits such as discounts on activity fees and aftercare fees. You are encouraged to complete this application as soon as possible.

Without an approved application by July 15, 2024, your family’s activity fee obligation will automatically revert to “PAYING,” and your child’s activity fee will be due in full.

To apply, please follow the below steps:

  1. Visit myschoolapps.com/Application
  2. In the box that asks for a zip code, enter 33431. Our school, AD Henderson University School/FAU High will appear.
  3. Click, SELECT DISTRICT & CONTINUE

As part of the NSLP application, you will need:

  1. The names and incomes of every member of your household.
  2. The school, grade, and birthdate of every member of your household.
  3. Your social security number and electronic signature.

To ensure only eligible families receive free or reduced activity fees, Florida Atlantic Laboratory Schools will request proof of income upon NSLP application processing. Below is a list of approved documents for demonstrating household income that will be required to determine free or reduced activity fees for A.D. Henderson University School or Florida Atlantic University High School.

  1. 1040 Federal Income Tax Form (PREFERRED): Please submit a copy listing the name of the child for whom the reduction is requested.
  2. Job Paycheck Stub: Provide a paycheck stub indicating the amount and frequency of pay received, a letter from your employer stating gross wages and payment frequency, relevant business papers if you are self-employed.
  3. Social Security, Pensions, or Retirement Letter: Submit a social security retirement benefit letter, statement of benefits received, or pension award notice.
  4. Unemployment, Disability, or Workers Compensation: Present a notice of eligibility from the State employment security office, a check stub, or a letter from the Workers Compensation office.
  5. Welfare Payment Letter: Include a benefit letter from the Florida Department of Children and Families.
  6. Child Support or Alimony Agreement: Provide a court decree, agreement, or copies of received checks.

When asked to provide documentation, please ensure they are clear and legible for speedy processing. Rest assured all submitted information will be handled with strict confidentiality and used solely for the purpose of determining eligibility for fee refunds and discounts.
